Replacement Document Forms

Do you need to re-order a membership or charter certificate?  Was there an error in the spelling of your initiate's name?  Has your charter certificate been misplaced?  Below you will find two forms:
  • Replacement Membership Document Order Form (for both certificates and membership cards)
  • Replacement Charter Certificate Order Form

PAYMENT
Pay by a single check or money order made payable to Alpha Kappa Delta.  Only institutional checks, checks cut from chapter's bank accounts, or money orders will be accepted.  If you cannot arrange one of these options, a single check cut from the faculty chapter representative’s personal account can be accepted.  Orders containing individual student checks will be returned.   Do not send cash.  If a business office is involved in the transaction, make sure both the order form and the payment reach the AKD office together.  We will not process orders until payment in full is received. We cannot accept credit cards as our Society is too small to set up this system.
Please make your check or money order payable to Alpha Kappa Delta.

COST
All replacement certificates are $10.00
Replacement cards are $5.00

Did AKD make the mistake in the spelling of a name?  The cost is on us! (Please be honest)

INSTRUCTIONS
Please read the instructions on the Excel Spreadsheet located on the first sheet. Please note that all certificates are mailed to the chapter representative to be signed. It is then his or her responsibility to ensure the certificate is sent to the correct party.
