STEP #1 Super Y League online registration can be found by following this link. This link allows players to pay for their club fees and to set contact information for club purposes. STEP #2 Once you have registered your player online, complete the Roster Information Form. This form can be found by following this link. STEP #3 There are a few other items that need to be submitted before players receive their player cards for Super Y League play. Super Y League Registration Form and Medical Release- Attached Below Please send Medical Form, Photo, and Birth Certificate via email to: STEP #4 Order Uniforms STEP #5 Send your $150 Coaching Payment postmarked no later than June 1st to Alliance FC 6757 Cascade Rd SE #56, Grand Rapids, MI 49546. Checks should be made out to your coach. You are also welcome to pay your coach directly. Payments after June 1st are $175. If you are not sure who to make the check out to please pay to Alliance FC and put your child's name and age group in the memo. |
