Below, under attachments, you will find documents that will help you in the registration process.

If your team is playing in the Grand Valley Soccer Association (GVSA), you will need to complete a GVSA registration form, submit a photo (2" x 2"), and birth certificate.

If you child is guest playing or is playing in an out of state tournament, you will need to complete a US Club Soccer form, submit a photo (2" x 2"), and birth certificate.


Refunds
Please note that for a number of reasons, we have a no refund policy.  In certain circumstances, we are able to offer a credit to be used for future programs.
