For New Patients referred to Active Care Physical Therapy, you will need the following forms completed for your Initial Evaluation. Please arrive 20 minutes early for your first appointment if you wish to fill the forms out in the clinic. Or you can use the printable versions below and have them completed prior to your visit.

You will need your insurance information, or a copy of your insurance card, for your first visit.
