    Welcome to the home of Active Care Physical Therapy in Roseville, California.  As the premier Physical Therapy provider in West Roseville, we are committed to meeting the needs of our community members and patients, as well as the quality expectations of our referring physicians.   
Active Care Physical Therapy is an involved partner in the community, helping to meet the physical therapy needs of Roseville and surrounding communities, as well as meeting the sports medicine needs of Woodcreek High School and William Jessup University. 
We have experience providing physical therapy for patients of all ages and abilities, including the Active Senior wanting to maintain or improve their quality of life.  Our professionals are also experienced in fitness program development, acute care for sports injuries, and progressive sport specific training.

Physician Referrals and Self Referral for Care

Physician Referral
A referral from your physician is needed, if insurance is to be involved in payment.  We accept referrals written on any prescription or referral form, as long as it is dated and signed by your physician. 
 
Patients do not need a referral from a physician in order to be seen by our physical therapist, if insurance will not be involved.  We provide self-pay options.
 
Work Comp patients must have an injury claim filed in order to be seen by our therapist and to bill Work Comp Insurance.
 
As a patient, you have the option to seek physical therapy care from the provider of your choice.  Active Care staff will take care of referrals, authorizations and insurance billing as a courtesy to our patients.  We will make every effort to make it easy for you, as our patients and customers, to access the caring and experienced physical therapy providers for West Roseville.
 
Alternate ways we may begin the process of helping you to overcome or prevent injuries:
 
Athletic Injury Evaluations are offered, and may be performed by our therapists and athletic trainers, for acute athletic injuries.
 
Pre-Sport Assessments are offered for athletes prior to their sports season, to help them prevent injuries and improve their performance.
 
Individual Fitness Assessments are offered for all individuals to help them identify areas of weakness or deficit and guide them in beginning or restarting a personal fitness program.

Services Offered by Active Care Physical Therapy:

Education seminars and community education programs are available, and we can customize a training program or presentation for your groups needs and questions..

We are conveniently located on Foothills Blvd, between McAnally and Junction Blvd in West Roseville, in the California Family Fitness shopping center, near the Sierra Point Luxury Retirement community.  We are 2 .5 miles, or less than 10 minutes, from the Fitness Center and lodge at Sun City Roseville, less than a mile from Woodcreek and Roseville High Schools, and 5 miles from William Jessup University in West Rocklin.  By using Highway 65 and Washington Blvd, Active Care Physical Therapy is easilly accessed by residents of Sun City Lincoln Hills.  See Map & Directions

Monday, Wednesday and Friday:  8 am to 6 pm
Tuesday and Thursday:  8 am to 2 pm
Closed Saturday, Sunday and Major Holidays
 
Insurance
We are a Medicare Certified facility, and accept most commercial insurances.  Call for your specific insurance coverage. 
We also accept Auto Accident claims, Work Comp claims and Attorney Liens. 
We do not participate in any HMO plans.
 
We have a reduced fee plan for self pay patients.
