Do not submit the Transfer Records Request to the school you are withdrawing your child(ren) from - it should be submitted to us along with your application. We will submit it to the school. Please include a fax number for the school and double check the postal address.

Tennessee homeschoolers please note: You should submit the Transfer Records Request form (page 5) with your enrollment application if you filed a notice of intent to homeschool with the state through your LEA for 2008/2009 or this year 2009/2010. Do not list your home address as school address. Use the city or county LEA district name & address as the school name and address.  You will not be homeschooling under TCA 9-6-3050 so do NOT file a “Tennessee Department of Education Church-Related (Satellite) HomeSchool Registration Form”  if requested to do so by your LEA.

Tips for using the Adobe Reader typewriter tool:

1. Click on the link for the enrollment form you need.

2. The form should open a new Adobe Reader window.

3 .Click on the Typewriter tool button above the form then click in the form at a spot where you want to start typing (note that once you click the typewriter tool and click on a place to type, the typewriter tool is no longer selected).

4. Type where you clicked and use Tab or Space to move to other places on that same line where you need to type information.

5. When you have finished typing on that line, select the typewriter tool again, then click on the next line where you need to type. (If the first form field in the next line you need to type on starts at the same place as you started on the line you have just done, then you can use the Enter(Return) key to move to that spot without having to reselect the Typewriter tool.)

6. Continue until the form is completed.

7. If you have a digital signature, use it wherever a signature is called for. Otherwise just type in the name.

8. When finished print and  use Save As... in the File menu to keep a copy for your records. Please include the student's surname at the beginning of the file name  when you save this document.

9. Attach the completed form to an email and send to thefarmschool.satellitecampuses@gmail.com. One way to do this is to open the File menu in the completed form window and click Attach to Email... This will open an email using your computer's email software with the form already attached.

Our computer answering machine software program will answer the phone, play a message and then it will listen for the fax signal. Once it hears the signal it will switch to fax mode and receive the fax. Please don't send faxes before 8 am or after 9 pm cst.
