Please print the attached forms and return them completed to Registration night on Tuesday, August 18^th.

• Emergency form:  One per family.  Please make sure to include your Email address.
• Physical form:  students entering Grades; Preschool, Kindergarten, & 6^th.
• Dental Form:  students entering grades Preschool, Kindergarten, 2^nd,  and 6^th.
• Eye Examination:  students entering Kindergarten
• Internet Agreement:  one per student
• Permission to Publish form:  one per student
• Student Health History:  one per student
• Emergency Medical Authorization:  one per family
• Preferred Transportation Form:  one per student
• Locker policy:  students entering grade 6 and any new 7^th or 8^th graders
• Student Information Sheet:  Any new sibling or student
• Parent Volunteer Form:  All parents who wish to volunteer, must have one on file in the office.
• Supply List:  Does not need to be returned.

Please remember that the office needs to have a copy of birth certificate, baptismal certificate, and social security card for all new students, including siblings.
