Sample Practical Roster- Dave Rockney and I found out that the state takes the current practical roster and makes a copy for each student on it and the attaches the copy to their packet. To help reduce the copy time we made this form up. It is based off of the NREMT practical roster (a scantron form). If you have a random student testing with a class it gives the state all the contact information they need to reach the student. Samara Steele-IREMSC
