Band Patch Order Form
2009-2010 School Year
Welcome to our Band Patch Program! The Band Boosters are pleased to present this years’ band patches. Please review the Power Point attachment of the patches so you can see what the patches look like. Print the appropriate order form, fill it out and return it to school. Orders will be placed in early October and will take a few weeks to receive them. Orders placed after the deadline may not be filled, depending on overstock. It is very important that you return the form and money on time to ensure that you receive everything that you want. If your order is late, you may not receive your patches until next spring when another order is placed.
Freshman may order their numbers (‘13), their “Freshman” strip patch and any of the extracurricular band patches that they participated in.
Sophomores may order their numbers (‘12), their “Sophomore” strip patch, the OEHS letter patch, and any of the extracurricular band patches that they participated in. They can also order any of the previous years patches.
Juniors may order their numbers (’11), their “Junior” strip patch, the band logo patch, any of the extracurricular band patches that they participated in and any of the previous years patches.
Seniors may order their numbers (’10), their “Senior” strip patch, and any of the previous years patches.
The number patches are priced per pair, not each.
Extracurricular patches that are available: Marching Band, Color Guard, Drum Major, Jazz Band, Pep Band, Percussion Ensemble and Pit Orchestra.
Please fill out the order form and return to Mrs. O’Neill or Miss Jones with payment by Mon. Oct. 5, 2009. Checks may be made out to: OEHS Band Boosters. Please DO NOT pay with cash.
It is our hope that the patches will be delivered in time for the November concerts. If the patches arrive on time, they will be available for pick up and purchase on those nights.
We will hold another patch sale in May.
