Faith Pharmacy is only open on Saturdays from 9:00am to 12:00pm.  

Patients are asked to arrive before 9:00am.  

There is a total limit of 120 prescriptions filled by the pharmacy on Saturday mornings.  Any prescriptions over the limit of 120 will be filled that Saturday if time permits.  If time does not permit the filling of additional prescriptions, the patient will have to return the following Saturday to pick up their medications.

 

Patients must have a prescription, a referral sheet, and a completed medical history form.

Patients referred to Faith Pharmacy must have these three items completed before receiving assistance. The prescription must be from a local doctor or nurse practitioner. The referral sheet is the map with several lines of information at the bottom that must be completed by the social or health clinic worker referring the patient to Faith Pharmacy. The medical history form should also be completed by the referring agency.

Referring agencies are asked to check the formulary to verify that Faith Pharmacy carries the patient's medication(s).  A list of drugs is available on this web site under Formulary.  

Some prescriptions can be rewritten for drugs of the same class (for example, Prinivil and Zestril are interchangeable).  If the prescription is written for a drug not available at Faith Pharmacy, please have the patient contact the doctor or nurse practitioner to see if they would consider switching the written prescription for a drug on the formulary.

Since Faith Pharmacy is only open on Saturday mornings and most doctors are not available at that time for call-ins, a change in prescription must be done before the weekend. Any changes by phone to a prescription hard copy must be done between a doctor and a licensed pharmacist. 
 

Narcotics or controlled substances are not available at Faith Pharmacy.

In addition to these classes of drugs, Faith Pharmacy does not carry prenatal, birth control, or antibiotics medications.

Patients must not be on a prescription plan or carry health insurance.

Faith Pharmacy is a short-term medical assistance program.

Faith Pharmacy will fill a patient's prescription three times only.. While the prescription is being filled, volunteers will provide help to patients in completing the forms required to enroll in free medication programs offered by various drug companies. These programs can provide up to a three month supply of certain medications. The drugs are mailed to the doctor's office. These programs vary in their requirements and usually have refill limitations. Case workers referring patients to Faith Pharmacy will also need to assist patients with completing the patient assistance program paperwork.
