Child Abuse and Criminal Record Check. This is a link that takes you to a Pennsylvania State web site where you can download the 2 clearance forms (CY-113 and SP4-164). Please review the Band Booster Bylaws to determine if you are required to obtain these clearances.

Fund Raiser Guidelines. Note that this document has several outdated references (eg Band director's name), but the basic information and guidelines presented are still valid and are to be followed. The document will be updated as time permits.

Fund Raiser Request Form. Mars School District document. The District approves all fund raisers and requires that this form is filled out and submitted to the High School office for all fund raisers prior to the start of the fund raiser.

| Parent Information collection Form. Use this for to provide your contact information to the Band Director and the Band Boosters. We will use this information to contact you in the event of an emergency and to convey Band and Band Booster related information to you. The information on this form will be distributed to the Band Director, Band Booster Officers and certain committee Heads as necessary.

Much of this information is contained in other documents or other places on this web site, but is condensed and summarized here for easy reference. Some of the information cannot be found anywhere else and can be considered "common knowledge" by parents that have been involved with the organization for a while.

Use of Facilities Sample Form. This is a sample form only. The actual form can be obtained at the High School office. It must be filled out prior to any usage of School District facilities (cafeteria, auditorium, parking lots, football field, etc...).
