Who can have Individualised Funding?
If you are already receiving services arranged by your NASC, contact your Service Facilitator at the NASC agency and tell them you are interested in individualised funding. Your Service Facilitator will help determine if you meet the eligibility criteria and, if you do, will then refer you to us.
You can only access IF if you meet the Ministry of Health's definition of disability, which is:
People with a lifelong disability of any age with physical, sensory or intellectual impairment (or a combination of these) who first access disability services before age 65, for whom the impairment(s) is expected to be at least 6 months duration and affect independent functioning to the extent that ongoing support is required.
You must also meet the following Ministry of Health eligibility criteria:

Have support needs that are within the 'high or very high' range in the NASC Guidelines

Have support needs that have been stable over a period of at least one year

Have a combined total level of support need that is not greater than full time residential care (as assessed by NASC)

Be assessed by a NASC as wanting to manage services through Individualised Funding payment

Be willing to comply with the requirements of Individualised Funding.
