FALL CREW REGISTRATION/REG PACKET

Hello Crew Families,

Well its that time again, Fall Crew Registration.

We will be having 3 Registration nights:

FIRST is Tuesday August 19th,  6:30-7:30 PM at the YMCA on Mechanic St. Manchester, in the Lobby, for all new incoming athletes.
This is the night that you will need to be swim tested which is mandatory. No athlete is allowed on the water without a successful swim test, so bring your swimsuit/towel and a $2.00 fee payable that night, to the YMCA which covers their Liability insurance.
 
SECOND registration night is also at the YMCA on Tuesday September 2nd,  6:30-7:00 PM in the Lobby (there is no swim testing on this night).
 
THIRD registration night will also be the night that we have our Mandatory Parent Meeting on Wednesday September 10th,  6:30 pm in the Library at Central High School. This is a very important meeting meant to inform our athletes and parents alike to all that will be going on this Fall Crew season, we expect to see all of you there.
 
Lastly, all registration forms are to be downloaded from this site, these forms need to filled out completely, including a valid physical, which will need to be valid through October 31st 2008. If it will expire prior to October 31st 2008 you must provide a doctor's note stating that your child is cleared for sports participation through October 31st 2008.

All these necessary forms along with the seasonal fee of $300.00 per athlete/per season constitute a complete registration packet.
If these packets are not 100% complete they will not be accepted.
These are the Club rules and they exist for the safety and protection of all of your children.

Important: Without a complete registration packet no athlete will be allowed to participate not even in practices!
Please get all necessary documents in place now. I would like to thank you all in advance for your cooperation with our registration process.

Please note:
  • To open this file you will need to have Adobe Reader on your machine.

2. Word Doc  version (If you have problems with the PDF file)
  • If you are using Internet Explorer as your browser just click on the link below and it should open for you.
  • If you are using Firefox then you will need to download the file.
    How to download in Firefox: Click on the link with your right mouse button (or hold the mouse button down if you are using a Mac), and select "Save to Disk" or "Save Target As" from the pop-up menu.
    You will be prompted to save the file somewhere on your hard drive, or the file may download automatically to your desktop.
