| This is an excel file that out EQ uses to keep track of HT. The first
sheet, the family report sheet, is filled out by a companionship
following visit. Most information is soon forgotten after a visit, so
this form hopefully will be filled out while the visit is fresh on the
Teacher's minds. Also, more information will be communicated on a
monthly basis, instead of just whether they were visited or not. The second form, the DL tracking sheet, is used by district leaders to track visits. It is submitted to the presidency via email or paper form. The third form is used for stewardship interviews between DLs and HTs. It includes notes and a plan for the future. The form is turned in to the presidency, and a copy is given to each HTer and the DL. This form will be used during the next interview for follow-up. - Submitted by Brian Boston |
