Please bring the following items to the May 18th LearnServe meeting:

• Your passport, which must have at least six (6) months of remaining validity and at least one blank visa page in it.
• Two copies of the visa application form. One of the copies may be a photocopy of the original application form.
• Two recent passport photos 2" x 2". Please sign the back of each photo and staple one photo to each application form.
• A visa fee payment of $100.00 (for U.S. citizens) for a multiple entry visa. (Only multiple entry visas, valid for three years, are issued to U.S. citizens.) LearnServe participants who are not U.S. citizens should consult www.zambiaembassy.org for the different visa entry options and fees. Only money orders and bank certified checks are accepted, payable to the Embassy of the Republic of Zambia.
