Please bring the following items to the May 18th LearnServe meeting:

• Your passport, which must have at least six (6) months of remaining validity and at least one blank visa page in it.
• Two photocopies of your passport's first page (where the personal information is written).
• Two copies of the visa application form.
Two recent 2" x 2" passport photos in color.
• A visa fee payment of $45.00 for a single (one) entry visa or $65.00 for a multiple entry visa. Payment can be made in cash, money order, or check payable to the Embassy of Paraguay.
• For student applicants: a completed and notarized parent approval form, along with a photocopy of your birth certificate and a photocopy of your parent(s)'s driver license(s).
