Request for permission to sell alcohol at your event, to be returned to the booking clerk who will seek approval from the Designated Premises Supervisor. Upon approval a signed copy will be returned to you. See reverse of form for guidance as to whether or not your event requires licensed 'sale by retail': providing drinks free to organisation members & raffles ordinarily does not, but it is always required if you intend to make a profit from the sale or include within ticket price. Please note the strict conditions regarding under age drinking.
