Time is running low for summer vacation. I hope everyone is having fun, staying healthy, eating right, and getting plenty of exercise. It is time to start thinking about EASING into the school routine. Set a time when bedtime will be earlier so that school wake-up time is not a shocker. It's time to start getting school supplies and uniforms ready. Make sure uniforms are clean and still fit. As the scout motto says, "always be prepared!"
Any student who may have a chronic condition or a condition needing medication or a plan at school, will be bringing home forms for next school year. Please take the time to get the forms completed during the summer so that they are ready for the start of school. Thanks in advance.
A "medication for school" form can be downloaded from the main page of the school web page: for Cardinal Pacelli, go to 'current families, online forms'. For Holy Family, go to 'Parent-Student Handbook, download medication form'. There is also a form attached at the bottom of this page as 'medication form'. Completed forms must accompany any medicine to be given at school. Most doctors also have generic school medication forms which are acceptable. Also remember that all medicines must be sent in the original containers (NO BAGGIES, PLEASE!!!) Pharmacies will provide an extra bottle for prescriptions for school if asked.
Please be sure that any medication you send in should really be given at school. If a med is to be given once, twice, or even three times a day, it is best to give it at home unless a specific time is listed on the prescription instructions (such as 'at lunchtime'). Three times a day can be morning, after school, and bedtime. Twice daily meds can be spaced out every 10-12 hours. It is so very important that instructions for medications be followed AS PRESCRIBED. This is why we want to have a form completed AND the original container at school. Thanks for your cooperation.

Attention parents of 6th graders
The State of Ohio has changed the immunization requirements for the 2010-11 school year. The changes will stand for every year after that. Students entering grade 7 will need a DPT booster of either Tdap (adolescent/adult) or Td (adult). If previously given, it must be within 5 years of entry to grade 7. Each 6th grader is bringing home a form to be completed by the doctor. You can also download the form which is attached below. (Dtap booster letter) The form needs to be on file at school before the start of school in August. Please send it in as soon as you have it completed. Thanks!
