Cast Docs - To Be Signed

* Please print, sign and return these documents to Nia or Karen by no later than April 1, 2009. 
* Everyone who is a part of the Flag must sign a permission slip/waiver and an acknowledgment of the rules and regs.      * There are two kinds of permission slips, for adults and minors.  Minors permission slips must be signed by a parent.
*For kids 11 and under, please remember to fill out one childcare form for each child who will be  in the cast or in  childcare at all during the flag, as well as a late night acknowledgment.
* If you have any questions, please call Nia at  ext. 122, or Karen at ext.118.
