Greetings!Welcome to the Parent Feedback form page. We are excited to offer this to our parents and feel it will be a wonderful tool for us as we strive to make every student-athletes experience at Cretin-Derham a positive one. The formatting may change on this page and we greatly appreciate your patience as we work through any technical gliches. The survey was written by the CDH Athletic department but is hosted and maintained by a third party vendor ensuring that all responses are completely anonymous. Below is a listing of all sports and levels available at Cretin-Derham Hall. Please click on your son/daughters sport and complete the survey. In an effort to collect the most accurate and beneficial information we ask that you please complete only one survey per student-athlete for the team/season your child is currently participating. Parents of Non-Varsity level student-athletes, the survey will be available for three weeks immediately following the completion of the season. Parents of Varsity level student-athletes, your survey will be emailed to you following the completion of your child's season. The email address used will be the same as listed on your emergency contact form on record with the CDH school office. If you do not receive a survey please inform the Athletic office and a survey will be emailed to you. Your results will still be completely anonymous. Parents whose child competes in the following: Alpine Ski Cross Country Dance (Performance & Competition) Golf Gymnastics Swim & Dive Track & Field Wrestling Your surveys will be emailed to you since both JV and Varsity practice together.
