General Athletic Information:

• Before every athletic season begins there are Parent/Athlete Meetings that detail general athletic rules and policies. It is highly recommended that parents and athletes attend these meetings for each season.

Athletic Forms

• Parent/Athlete Meeting Dates are posted below or by clicking on the link to your left.

• All Athletic Paperwork must be turned in before the season begins. All paperwork should be turned in at the Parent/Athlete Meeting.

• If paperwork is not completed and on file the athlete will not be able to practice or tryout.

• There are 5 forms that need to be on file each year in order to participate in athletics.
• The forms can be found here.
  • Physical Examination Form must be on file in the Athletic Office and current within the past three years prior to the start of athletic season.
    • Incoming Ninth Graders should turn in their physical examination forms by July 15th accompanied with all athletic paperwork.
    • Athletic Paperwork is not sent home in your registration packets you must print it from link to the left.
  • Emergency Form must be turned in at the beginning of every season of participation (this is new for 2007-08 school year) .
  • Chemical Agreement Form  must be turned once every year prior to the start of the season.
  • 2009-10 Minnesota State High School League Athletic Eligibility Form must be turned in once every year prior to the start of the season.
  • Annual Health Form Questionaire must be turned in once every year prior to the start of the season. This form is on the back of the MSHSL Form.
