Forms
- Class 1 and 2 medical form (this form is used for adults and Scouts under the age of 40. The Class 1 section must be reviewed and signed annually. THe class 2 section (doctor's physical section) is good for 3 years. If you plan on attending one of the high adventure areas (Philmont, Jamboree, Sea Base, or Northern Tier), then a class 3 form will have to be completed. These are generally their own special formatted form.
