Middle School Summer Reading Program

 

All incoming fifth grade students are to read the required book, The Adventures of Tom Sawyer by Mark Twain and complete the attached Book Report form. Additional copies of the form can be found on Mrs. Garner’s web site through www.bettonhills.com. Students should be prepared to discuss the book the first week of school and will take an exam on Friday, August 28^th. The Book Report form will be their first grade for the 2009-2010 school year.

 

Instructions for completing the Book Report form:

 

• All responses should be completed neatly in cursive.
• Use complete sentences using the 5 W’s.
• Pay attention to mechanics
• Capitalization
• Punctuation      
• Grammar.
• Spelling

 

Extra Credit:

 

For additional books that are read from the reading list, students may receive extra credit. To do so, they must follow the reporting instructions below for each additional book read.

 

Please follow the reporting instructions carefully:

• Create one note card for the additional book you choose.
• Use 5" x 8" cards. (Points will be deducted for 4” x 6" cards)
• Write neatly in ink or type your cards.

 

ON THE FRONT OF EACH CARD INCLUDE:

• Title (underlined) Author
• Your name Date you finished the book
• Setting (time and place of action)
• Names of major characters
• Category (fiction or non-fiction)
• Type of book ( mystery, romance, adventure, biography, history, science fiction)

 

ON THE BACK OF THE CARD INCLUDE:

• Would you recommend this book to others? Why or Why not?
• Be sure to respond in no less than four complete sentences.
